sugar has been linked to obesity, diabetes, and tooth decay and probably other diseases.

Personally as long as you only have a few cups of coffee a day the sweetener is not a big issue compare to other things yo drink and consume.
i use aspartame and I think that caffeine in coffee is probably a greater health risk.
