The worst part is that I've done it twice...

A customer of mine poured water into the grinder on their automatic machine. it wasn't good news for the machine.
The design of the particular machine has two lifting panels on the top of the machine which look exactly the same. It wouldn't be hard to mix them up early in the morning.
