    Also those who grew up on instant coffee seem to share the obsession with having it served scalding hot.

    In country towns it's not uncommon even now to receive scalding hot coffee from cafes.

    They need to widely advertise their 'superheated' coffee as all the pensioners from miles around could call in for their cup of scalding coffee. Then they wouldn't have to badger the Barista to "...make it extra hot".
    I took friends from out of town to a favourite cafe and they pulled that one on the Barista - who looked at them like they were idiots. I suggested that they shouldn't do that as coffee was not tea and wasn't intended to be served boiling hot. They were long-term tea drinkers and I think this behaviour stems from an obsession of some tea-drinkers with scalding-hot drinks.

    The hot water tap on the machine is very hot water, unless there’s a mixing valve which mixes in some cold to get it to around 80C. Some high end domestic machines have this (R91 and GS3).

    On the taint comment, in a busy cafe it’s unlikely to be tainted.

    They would be (or should be) turning over the service boiler water every day, or even more than once a day.

    Correct. It’s usually because the cafe gets its hot water for tea and coffee from the espresso machine. Which is usually an HX with the boiler running hot enough to maintain steam power through busy periods. This is a terrible idea as the water from the espresso machine is both too hot and often has a tainted flavour. Runs and water towers are relatively cheap and there’s no excuse for cafes to not have one really.
