    The closest we Aussies have come was the renaming of the Department of Immigration and Multicultural Affairs to the Department of Immigration and Citizenship - but they included the A from and in the acronym to avoid any embarrassment...

    Its quite rampant in management meetings all over the world. Its just that most places catch the howlers before they get to the public stage.
    Some years ago I worked for a company who had significant dealings with the public.
    They wanted to put in a process to look after their high value customers (good idea), so they started a project to build a dedicated database (good idea) which tracked sales data, trends, preferences etc. (good idea).

    All IT were invited to the kick-off meeting where they announced that this system would be called the Customer Relationships and Procedures project. (bad idea).

    When asked if anyone was concerned that the acronym for this was CRAP, management just faffed around and made comments like "Oh its just an internal name", "Oh, no-one else would think that right away, youre just being a nuisance" etc.

    No-one likes a glaring cock-up pointed out to them, but they quietly changed the name a few days later.

    Would have been lovely to see the press generated from that if it went ahead:
    "[company name] launches CRAP project".

    This is the whole reason Dilbert is successful. Often, its only a slight exaggertation, and sometimes, not at all.
